By signing this Client Agreement, you understand that WheelHouse Yoga and its agents assume no responsibility for your personal health. You acknowledge that you have disclosed all information regarding your health to WheelHouse Yoga and its agents . You further understand that WheelHouse Yoga and its agents are not medical professionals and cannot diagnose or treat any medical symptoms or ailments. You are encouraged to seek the advice of a medical professional prior to engaging in any physical activity.

I fully understand that I may injure myself as a result of my enrollment and
subsequent participation in fitness programs at WheelHouse Yoga. I, hereby release WheelHouse Yoga and its agents from any liability now or in the future for conditions that I may obtain. These conditions may include, but are not limited to, heart attacks,
muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat
prostration, injuries to knees, injuries to back, injuries to foot, or any other illness or soreness that I may incur, including death.
